Employment Verification Forms: Complete each form attached to the links below. Save the completed form and upload them below. Please understand that you will not be allowed to start work, until these documents have been received.

New Hire Acknowledgement

Authorization: Please read, review, and sign.

I hereby authorize EHS and its designated agents and representatives to conduct a comprehensive review of my background through a consumer report and/or and investigative consumer report to be generated for employment.

Acknowledgment of Payroll Deductions: The information provided below is to make you aware of the possible payroll deductions that may be applied to the employee, should EHS perform the service. Employee's are only obligated to pay any listed amount, if services are provided by EHS. Please check the box, to acknowledge the amount. This is for informational purposes only, and prices are subject to change.

Please note: Any charges under $100.00 will be deducted out of one (1) paycheck. By acknowledging, you authorize EHS to deduct the agreed upon amount, if the listed service(s) has been provided.
